At Cleveland Clinic we pride ourselves on being able to provide the best care possible for our patients. Due to increasing demand we are unfortunately unable to take on new patients at the current time. We hope to offer our wide range of services to new patients again soon.
Unfortunately we are not offering services to visitors either at the current time.
Should you need to make any changes to your personal information, an amendment form must be completed and handed into one of our surgeries. You will be asked to show identification to confirm it is you, so please ensure you attend in person and bring along copies of your photographic ID and proof of address (within 3m). This is a legal Data Protection requirement.
You can also update your contact and address details by visiting one.gov.je which is a secure government portal that communicates directly with the surgery safely.
Should you need to have access to your records, please complete a Subject Access Form (SAR) and hand it into one of our surgeries along with proof of identification to confirm it is you.
Please download and complete the appropriate form from below. You'll need to print it out, sign and return it to us in person along with copies of your photographic ID and proof of address (within 3m). You can also pick up a form from any of our surgeries, or request an email version is sent to you by dropping us a line at town@clevelandclinic.gpnet.je
